Added a new Doctor or changed locations? No problem! For your convenience, we offer updates to any previously-purchased digital forms at a fraction of the original price. Once you place a new order to request updates, you’ll receive a link to the customization survey once again. Please complete that survey, and then we’ll send you the same form or forms that were originally purchased within 5 business days.

Important – All Digital Forms UPDATES require an original purchase of digital forms. Any NEW digital form orders placed incorrectly that are not associated with a previous order will be refunded, and access to the materials will not be approved. If this is your first time purchasing digital forms from WAP, please view all options here.
